Workplace Injury- Team Member SOP

Policy/Purpose:  Any workplace injury should follow the procedure listed below to ensure the health and safety of all our team members, and also make sure that we have reported everything properly, especially in case of workers compensation needs and timely processing of any and all paperwork.

 

Procedures:

  1. The manager must be alerted immediately to a team member’s physical injury.

  

  1. The manager must assist the team member with the injury and offer medical attention immediately.  Please remember, we are not medical professionals, so we should not advise anyone what they should do, but make them aware of their choices and offer medical assistance.
    1. If the team member accepts medical attention, the team member must be able to get to the clinic/hospital on their own or from someone they know, or in the case of an emergency, the manager must either find a way for the team member to get to the clinic/hospital or call an ambulance.  *The manager must alert the General Manager/Sous Chef/Operations Manager, COO and Ownership through a Basecamp message (Required Notification).  
    2. If the team member declines medical attention, the manager must ask them multiple times, based on the severity of the injury.  If the injury is severe enough for the team member needing to go home from work, it is likely severe enough that they should seek medical attention. 

 

  1. The manager must fill out the Injury Report Form (Found in the Drive under PDF Folder, Forms; also listed below) with as much detail as possible.  The manager should ask other staff that were witness to the incident to gain as much real-time knowledge as possible.  
    1. If the team member seeks medical attention, the manager needs to send the injury form with the team member, or send the injury form to the clinic or hospital the team member is going to.
      1. The manager should alert the clinic/hospital that the team member is going to in case they need additional information and also so they have a point of contact should they need.
      2. If the team member seeks medical attention, the manager must also fill out a Form I9 (Found in the Drive under PDF Folder, Forms; also listed below)
    2. In the event that a team member does seek medical attention, any forms given to the team member from the clinic or hospital should be copied and attached to the injury form before submitting.
    3. the injury form should be filled out regardless of whether the team member seeks medical attention

 

  1. If the team member does not seek medical attention the manager can submit the injury form and any additional paperwork to the HR Manager. 

 

  1. If the team member does seek medical attention the injury forms with the Form I9 and any additional paperwork needs to be submitted to the HR Manager.  The MR Manager will submit the paperwork to the insurance company, and then they will put it in the team member’s file for reference as needed.

 

  1. The manager must follow up with the team member the next day to ensure that they are ok and can come back to work.  If the team member is unable to work, they must cover their shifts per the attendance policy.  If the team member went to a clinic or hospital, they must provide a doctor’s note releasing them to work, listing time needed to be away from work, or any restrictions if allowed to go back to work.

 

  1. The manager must include this information in the Daily Management Log.
